What is a Driver Qualification File?
A DQF is a mandatory record-keeping requirement that proves your drivers are legally, physically, and professionally qualified to operate a Commercial Motor Vehicle (CMV). Whether you have one truck or a hundred, the DOT requires these files to be complete before a driver ever hits the road.

The 2026 DQF Compliance Checklist
To stay ahead of DOT auditors, ensure every file contains these eight essential components:
DOT-Compliant Employment Application
Motor Vehicle Record (MVR) – Initial & Annual
Safety Performance History (Background Check)
Road Test Certificate
Medical Examiner’s Certificate (MEC)
Certificate of Violations (Annual)
Drug & Alcohol Clearinghouse Queries

- DOT-Compliant Employment Application
A standard resume isn’t enough. The application must include:
⦁ A 10-year employment history for safety-sensitive positions.
⦁ Personal details, including SSN and date of birth.
⦁ A signed statement certifying all information is true. - Motor Vehicle Record (MVR) – Initial & Annual
⦁ At Hire: You must pull an MVR from every state where the driver held a license in the previous 3 years.
⦁ Annually: You are required to pull a new MVR every 12 months to review the driver’s continued fitness for duty. - Safety Performance History (Background Check)
You must investigate the last 3 years of a driver’s employment history. This includes reaching out to previous employers to check for accident history and drug/alcohol violations. - Road Test Certificate
Drivers must pass a road test on the specific type of equipment they will be operating. While a valid CDL can sometimes serve as an equivalent, keeping a specific road test certificate is a “best practice” that adds an extra layer of audit protection. - Medical Examiner’s Certificate (MEC)
In 2026, the transition to digital medical certifications is in full swing. However, carriers must still ensure their drivers are examined by a professional on the National Registry of Certified Medical Examiners and keep a copy of the “medical card” in the file. - Certificate of Violations (Annual)
Once every 12 months, the driver must provide a list of any moving violations they received. The carrier must then review this list against the annual MVR. - Drug & Alcohol Clearinghouse Queries
Before hire, you must conduct a Full Query in the FMCSA Clearinghouse. Additionally, you are required to run a Limited Query at least once per year for all active drivers. - Skill Performance Evaluation (If Applicable)
If a driver has a physical impairment that requires specialized equipment, a valid SPE certificate must be present in the DQF.
